Abstract

The main objective of this systematic review is to report whether there is a considerable reduction in morbidity and mortality in patients with Acute Myocardial Infarction with ST-segment elevation (STEMI) submitted to primary angioplasty compared to thrombolysis. For this review some published studies were selected from these electronic databases: PubMed, Medline, Lilacs, SciELO and Bireme. Based on the 12 articles used, a comparison of the two treatment strategies was performed with a focus on 30-day mortality (primary outcome). The final sample consisted of 2,470 patients. Following the care protocol, fibrinolysis was provided to 243 patients (9.8% - fibrinolysis group) and 2227 patients were transferred to a PCI center (90.2% - PCI transfer group). The average time for PCI was 119 minutes. Mortality tended to be lower in the PCI x Fibrinolysis group (5.1% x 7.7%, P = 0.09). However, when evaluating patients transferred within 99 minutes until PCI, mortality was 2.0% (P<0.01) and between 99 and 140 minutes was 4.6% (P = 0.03), significantly lower in the PCI group. In patients who underwent thrombolysis, there is a reduction in mortality of 7.7% (p < 0.03), as well as a reduction of 52 minutes in treatment time when compared with PCI and, consequently, a significant reduction in long-term mortality. Therefore, what will influence the choice of procedure is time.
